Class Registration Forms:
Pre-registration is required for classes that are offered as sessions. The form may be mailed to Premier or dropped off at the clinic. If paying by credit card the form may be faxed, or scanned and emailed to Premier.
New Patient Forms:
New patients may follow the below link to print a history and registration form. Along with the completed forms, we ask that insurance cards, photo identification and a list of all current medications including supplements be provided at the first appointment.
